COVID-19 Vaccine

Since 1973, Montclair Hospital Medical Center has provided high quality care to our community. We are honored to partner with the state to continue to keep you safe and healthy during such critical times by offering COVID-19 vaccines starting in early April through July 7, 2021. Starting on July 15, 2021, we will only be offering Moderna vaccines to adults aged 18 and older from 8:00 a.m. to 1:00 p.m in the front lobby of the hospital. Walk-ins are accepted, however appointments can be made through MyTurn. We request an ID card and insurance card if available at time of vaccination.

We encourage every member of the Montclair community to get vaccinated when it becomes available to you. Check your eligibility and schedule an appointment at a location near you by visiting or by calling 1.877.813.7312.

We support the efforts being exercised within San Bernardino County to keep the community safe and healthy. We encourage receiving the vaccine as your tier is called to continue the fight against COVID-19. While the COVID-19 vaccine is a crucial resource to help the community of Montclair transition back to "normal," it does not mean the pandemic is over. Montclair Hospital Medical Center would like to remind you to continue social distancing, avoid contact with those who have been exposed or are confirmed positive of COVID-19, to properly wear a face mask in public and practice hand hygiene as directed by CDC and the State of California to help slow and stop the spread of COVID-19.
